It is India's second medal in the Rio Games after wrestler Sakshi Malik bagged bronze in the women's 58 kilogram category on Wednesday.
In a hard-fought summit clash, which lasted one hour and 20 minutes, fortunes fluctuated dramatically at the Riocentro - Pavilion 4.
With the win, two-time World Championship bronze medallist Sindhu now trails Carolina 2-5 in career head to head meetings.
Sindhu became the first Indian woman to win silver at any discipline at the Olympics. Sindhu is also the fourth Indian individual Silver medallist, joining shooters Rajyavardhan Singh Rathore (2004 Athens), Vijay Kumar (2012 London) and Sushil Kumar (2012 London).
The Indian shuttler started on a positive note but after 3-3, the Spaniard dominated the proceedings and took four consecutive points to lead 7-3 and then 9-5.
Unperturbed by the pressure of the occasion and her towering opponent, Sindhu controlled her nerves and displayed swift court movements and hit bodyline smashes to diminish the lead to 13-15.
World No.10 Sindhu, who was trailing 16-19 at one point, fought her heart out and took five points on the trot to claim the first game in 27 minutes.

Andhra Pradesh Chief Minister N Chandrababu Naidu laid foundation stone for the 'AP MedTech Zone' at Pedagantyada. The facility, which will come up in 270 acres is likely to be operational in a year.
The Chief Minister said about 80 per cent of medical needs are being imported from other areas and establishing more such parks will definitely bring down the costs up to 40 percent.
He said 40 companies have already expressed interest before laying of the foundation stone.
"Around 200 companies would come in the park and could generate employment to around 25,000 persons", Naidu added.
6. Bolt gets 9th gold with another runaway win
Jamaican athlete Usain Bolt became the first athlete to 3 gold medals in Olympics by winning a gold medal in 4X100 m relay race in Rio Olympics.
Usain Bolt bid a blazing-fast farewell to the Rio de Janeiro Games -- and likely the Olympics altogether -- yesterday night with yet another anchor leg for the ages. He turned a close 4x100 relay race against Japan and the United States into a never-a-doubt runaway, helping Jamaica cross the line in 37.27.
Japan won the silver medal, finishing .33 seconds behind.
The U.S. finished the race third but endured yet another relay debacle -- disqualified because leadoff runner Mike Rodgers passed the baton to Justin Gatlin outside the exchange zone.
That promoted Canada to the bronze medal.
This marked the ninth time since 1995 the U.S. men have been disqualified or failed to get the baton around at Olympics or world championships. That will cause more hand-wringing in the States.
